The Phase 2 NEJM trial by Jastreboff et al. (2023) found that Retatrutide's most significant appetite and weight effects occurred at higher doses (8 mg and 12 mg weekly), with the escalation protocol taking 24 weeks to reach the target. At lower doses, appetite suppression exists but is milder.

If you are early in a protocol and at a lower dose, this is the most common reason for persistent hunger.

2. Adaptation to the Initial Effect

Like all GLP-1 receptor agonists, the subjective appetite suppression is often most pronounced in the first days at any new dose. As the body adapts over one to two weeks, some of this acute effect diminishes before stabilising. This can feel like the compound has "stopped working" even when it has not.

3. Diet Composition

However, the appetite reduction was not uniform: some participants in lower-dose groups reported minimal appetite change early on, while others showed strong responses at the same dose.

Importantly, the trial used a structured dose escalation over 24 weeks specifically because higher doses early produced greater gastrointestinal side effects. The hunger reduction builds as the dose builds.
